Clinical Effect of Transcutaneous Electrical Nerve Stimulation (TENS) on Anxiety
and Pain Management in Pediatric Dentistry

Abstract
Dental anxiety in children is a significant factor contributing to missed dental
appointments and poor oral health outcomes. Negative dental experiences during
childhood can lead to the development of dental phobia in adulthood, a severe and
persistent fear that often results in avoidance or postponement of dental visits. This
behavior not only worsens oral conditions such as caries and periodontal disease but
also increases the risk of systemic complications, including cardiovascular and
metabolic disorders. Early exposure to stress and fear in dental settings can therefore
have long-lasting consequences on both oral and general health.
In recent years, there has been a growing shift in dental and medical practice toward
discovering and applying new non-pharmacological methods for managing anxiety,
stress, and pain during dental procedures. These approaches aim to minimize or
completely avoid the use of local anesthesia, particularly in patients with a pronounced
fear of needles and injections. Techniques such as behavioral distraction, virtual reality,
relaxation training, and sensory modulation have shown promising results in improving
patient cooperation and overall comfort.
Among these innovative techniques, transcutaneous electrical nerve stimulation (TENS)
has gained attention as an effective anxiolytic tool in pediatric dentistry. TENS works by
delivering mild electrical impulses through the skin to modulate pain perception and
induce relaxation. Studies have demonstrated that TENS can significantly reduce
anxiety and discomfort in children undergoing dental treatment, offering a safe and non- invasive alternative to traditional anesthesia. Its integration into pediatric dental practice holds great potential for improving patient experience and long-term oral health outcomes.
